Student Initiated Withdrawal Form

If you are in a late start class please stop in the records office to withdraw from your class.

The withdrawal period has ended for full semester classes. If you are a student requesting a withdrawal from a second eight week class, a withdrawal form can be completed in person at IVCC's Main Campus in the Registration area in Student Services (CTC101) or at the Ottawa Center. Photo ID is required to submit the withdrawal request.
